There is an additional $10 fee for completion of forms for use outside of the patient's medical record, including:

Medical record $25.00 for first 25 pages and 50¢ per additional page.

Appointments must be cancelled within 24hrs. We enforce a "no show" policy which may include possible dismissal from our practice.
